A study was published in 2004 mentioned that 5.6% of the stroke patients who were on PEG feed have aspirated. I am still seeing patients on PEG develop aspiration pneumonia. What are your thoughts on

A study was published in 2004 mentioned that 5.6% of the stroke patients who were on PEG feed have aspirated. I am still seeing patients on PEG develop aspiration pneumonia. What are your thoughts on this subject?.

As a GP I look after a number of post CVA patients who come home from hospital with peg tube in because they cant swallow. When dealing with a treatment I sometimes think how would I feel if I or one of my relatives of the appropriate age had that treatment. With patients who are not going to get better, then I think if I were the patient, I would not want the PEG. But it is difficult to say if you are not in the situation. Some of my elderly post CVA patients have lived for very many years in Nursing Homes with pegs in situ. Some of these patients have died of 'pneumonia' but I dont think it has been aspiration pneumonia, but they would not have had PMs so I cannot be sure.

This is an important subject which raise issues about the quality of the lives of the patients in addition to the subject of early detection of dysphagia and aspiration risk in stroke patients. From practical point of view the patients need speech and language therapist or (SALT) to test swallowing but SALT professionals might not be available and the junior doctors or the specialist nurse might take the responsibilty of bedside testing for dysphagia. 
I do not think that PEG feed is for every dysphagic stroke patients. I came last week across a sudy which concluded that " Clinicians should be cautious about PEG placement for stroke patients with severe dysphagia who were independent prior to the stroke, aged ≤80 years, and show NIHSS score ≤17 on day 10, because their swallowing dysfunction may improve in a few months". The debate about this issue in stroke medicine is useful.

aspiration may occur despite peg. positioning and chest phygiotherapy best option to prevent this.
