I read with interest the

(Systematic Review of Randomized Controlled Trials of Carotid Endarterectomy for Symptomatic Carotid Stenosis). It concludes that: 

Endarterectomy is of some benefit for 50% to 69% symptomatic stenosis and highly beneficial for 70% to 99% stenosis without near occlusion. Benefit in patients with carotid near occlusion is marginal in the short term and uncertain in the long term.
What is about your practice locally?..our vascular surgeons are keen to see patients with 70% stenosis and above but are not so keen if the stenosis degree is less than that.

if patient developed recurrent ischemic event despite conservative treatment, marginal stenosis can go endarerectomy.
