Pneumothorax is a real medical emergency & I also feel it horrible but a challenging situation.
It is always helpful to have assistance of another competent pulmonologist or anesthesiologist.

...Or a thoracic surgeon !   ( i asked for a thoracic surgeon advise once when i was alone during a night shift , and wasn't actually sure if it was a compressive pneumothorax or a giant bulla )  

PS : i'm new on this forum and i'm using french language in medicine  , so please excuse my english , i'm trying to improve it  .

I also don't know if i can post  interesting CT scans  about the case  i've mentioned .

We have interesting  pneumothox cases   at the pulmonology service of Bab El Oued university  Hospital  , Algiers , ALGERIA  .

As Dr. K. says a pneuomothorax can be a real medical emergency. For me, being 125 miles away from my nearest general hospital, it's always a bit of a worry. But, like any other emergency, if keep calm and you follow your ABCs you won't go far wrong. So far in nearly 20 years of remote rural practice I have never had to put in a thoracostomy tube and can only recall once decompressing a possible tension with a large bore needle. If you are really worried just make sure you get good training in putting in thoracostomy tubes - if you can at least do that you will do just fine and so will your patient.

i'm used to drain;  it's not a problem ;  but  i'm always stressed  specially before  placing the chest tube .. !

I am a UK GP. I have not inserted a chest drain since I did my first house job at a 'Chest Hospital' over 30 years ago . I can still remember inserting chest drains. I don't like hurting people and they are very big needles!. I was not very keen on doing lumbar puncture on children with suspected meningitis either, in those days we did them, No patients had complications , but I and probably the patients did not enjoy the procedure. As a GP  I used to insert 'Zoladex' [Goserelin, a long acting gonadorelin analogue depot] implant in a few prostatic cancer patients on home visits and I did not like doing these, big needles although not the size of chest drains. The district nurses  do these depot injections now. I was definitely not 'cut out' to be a surgeon! I prefer the non surgical bits of medicine
So no it is normal to feel stressed in front of a pneumothorax !  a pat on the back if you hit air and deflate the pneumothorax.

I had a tension pneumothorax on a thoracic ward round as a trainee in thoracic med. I diagnosed it myself.

The ICC went in without local anaesthetic. They were in a hurry. I was very dyspnoeic. My chest was bruised for weeks.

I always carried a large bore IV set in my car in case for a few years after that. I never needed it but had another one ten years later and had surgery for it.

Nothing phases me. I work in slow motion in catastrophes. I have been at a dozen serious car crashes. The time-space continuum slows down for me while others fluff around in chaos.

I always feel calm in catastrophes. It is a Greek word. The first Greek god was Chaos. Hades is very patient and Hope is more impatient but persuasive,
