This month's Thorax has a paper on A1AT deficiency, spcifically looking at A1AT levels in the general population. It's an interesting paper, worth a read: The authors discuss the use of replacement therapy. We have never given it to anyone in our practice here in Dundee. Recently a patient asked for it, and presented us with a swathe of information on its efficacy, along with recommendations on its use. Our medicines management committee declined the request. Have you ever given A1AT replacement therapy? Would you? Could you? Should you?

I had a patient who needed it. We found out it was $500,000 per annum. He didn't have the money. I didn't either. He kept on having his aortic valve replacements becoming incompetent. He had emphysema as well of course. I figured it was because of the unopposed trypsin breaking down his valve ring. The surgeon had not thought why this kept happening. He came to my rooms one day complaining he was very breathless. I listened to his chest. He had a new loud EDM murmur. I rang the surgeon. They replaced his new valve the next day. The rest is history. Moral: Have a genetic deficiency which is cheap. It is a sad story. |
