This uterus can confound an expert sonologist

What can you opine from these images?
This is a young lady with pelvic pain.
These are transabdominal ultrasound images. Transvaginal images will follow:
What is that in the right adnexal region (right of the uterus.. on the left side of the image):
This was the main finding---



And what is that in front of the body of uterus? Do you see some cystic lesions there?

The radar screen shows a whale is about to hit the boat from underneath. Could it be that big white one again? 

As a sailor, I would be getting the rubber ducky ready and start looking for my life jacket.

Ok, not a whale. How about a Manta ray or a sunfish? 

Please put me out of my misery, Joe. you know for us non-sonographers, sound waves look like radar and I would prefer radar any day. I can use CT scans but to me a sonar is something dynamic like the ping ping on a submarine. Looks a but faecal to me.

OK...I will reduce your misery of you two (non sonologist) mortals..he he... it is a solid mass to the right of the uterus....do you see that white rim along the solid mass? What can cause this in a middle aged female?.. Think think.. then I shall proceed to solve the rest of the mystery...

What can be the cause of increased density along the rim of a solid mass near the uterus...ok. . this deposit is hyperdense on CT (and hyperechoic on Ultrasound)....

An ovarian tumor? What? An extra uterine old calcified pregnancy? It is not a myoma, right? it doesn't seem part of the uterus.

How about either a calcified sunfish as it has been there a long enough for dystrophic calcification, an aneurysm of the internal iliac or one of those red things in the pelvis or calcified Chaga's disease in a Peruvian guinea pig. 

Please put us out of our misery as I am in the rubber ducky now with Yoram with life jackets on and looking at fins circling our boat.

Just try knifing those big bad fishes...ha ha...
Odysseus.. fibroids can occur in the broad ligament too..with a short stalk attaching them to the uterus.. that round mass is a broad ligament fibroid with calcification along the rim.....Nowwhat is that cystic looking lesion ? (all those small black spots...in this image)?
same patient.. use your harpoon.. knock those sharkies.. ha ha

A dermoid with a set of dentures, teratoma with teeth, a cystadenoma with M&M's  or a trichobezoar that took a right turn out of the sgmoid colon and got lost.. Am I close?
