This was a patient who came for a follow up ultrasound. The diagnosis is obvious, but there is more to it than meets the eye, or is there? What do you make of this image of the uterus? Joe.

Hello Drjoea

The first on mind is adenomiosis of uterus. Though when i enlarged the picture it arises suspicion that it can be bicornuate uterus with pregnancy in one of the cavity.

Ok.. This was better seen on real time imaging (the sonologist always has an advantage over those viewing still images).. ;-)
This is a common condition of the uterus.. the image shows a mass lesion.....yes "Hip radiologist".. in the posterior wall of body...
Can you explain the appearances now?
I would rule out adenomyosis as the lesion is too well defined and echogenic, well encapsulated to be adenomyosis.....
Joe.

Hello Dr Joe
Ok then I suppose that there is a myoma of uterus and this appearance in the posterior wall of uterus is Ovary with the follicles. Yes It's hard to discuss these photos from a  distance....Sofiko

That is correct....Hip radiologist......there is a fibroid of the posterior uterine wall with degenerative changes within it.....all those dark (hypoechoic areas) are areas of degenerative changes.
Joe.

Thanks Joe... a little away from my area of expertise! Was the patient symptomatic or was this a purely incidental finding?

This patient had discomfort and pain in the pelvic area.....Most of these patients ignore their symptoms until it gets pretty bad...
Joe.
