Does anyone care about public health?

The latest white paper has come out on public health and the papers are full of comments from people telling the govt where to put their nanny state. Most doctors don't seem to care that much about public health. The govt is actually saying that McDonalds etc can set their own levels of fat and salt (people say this is like putting Dracula in charge of the blood bank) but is also setting up Public Health England which could have £4bn to spend on public health measures. GPs may get extra money for encouraging patients to lose weight. It's a bit same old, same old- it's just too difficult to get anyone to eat less fat or drink less alcohol because they like doing it.

I think it can be easy to feel disempowered about public health, because the issue is so vast - all aspects of health, for the whole population - where to begin?

As doctors we often just have people's problems in bitesize chunks. When we get all somebody's problems at once, it can feel overwhelming (?"heartsink" patients).

"It is better to light a candle than curse the darkness". The wider determinants of health are extensive and complex, but we should avoid all-or-nothing thinking - we can't make a dramatic difference, so what's the point? Through our efforts for our responsible populations, we can and do make a difference, often for the most disadvantaged in society, even if it may not be as great as we might want.

The public health services started in US when the spread of disease was a serious concern here. The PHS officers became very much concerned about the importation of disease into the United States. The PHS officers worked directly with state and local departments of health to track, prevent and arrest epidemics as they emerged. They helped the public when everybody was battling with influenza, polio, typhus, typhoid, smallpox and a range of other. Yes in the US it is a big concern.   And by all rights it should be,   Lack of public health concern, equates, to malnutrition,  Disease spread,  Proclivity of antibiotic resistant strains of bacteria, and lack of proper medical treatment for other wise easily treated maladies.
