In America,antidepressants are increasingly prescribed for people who have not been diagnosed with an actual psychiatric disorder, according to a new study that raises questions about whether or not the drugs are being prescribed inappropriately or too often.According to the new study, published Thursday in the journal Health Affairs, nearly three-quarters of prescriptions written for antidepressants in 2007 came from non-psychiatrists, About 4,000 patients who did have a mental health diagnosis received the drugs from non-psychiatrists in the study period. |

I think this could apply to any specialty . I would say that the basic rule of safe prescribing should apply - only prescribe within areas of your competence and with drugs you are famililar with, Do we really want to see a medical world where only a specialist can prescribe drugs? |

Agree with sadian - we don't have a 'right', we are able to prescribe and administer any drug which we are competent to prescribe. If you aren't competent then you shouldn't be doing either. Look at where that got Dr.Conrad Murray, Micahel Jackson's 'physician'. John |

I also do agree with Sadian in that every physician experienced in prescribing psychiatric medications may prescribe the drugs provided we have a good knowledge & a competence to prescribe the psychotropic agents. We should refer the patients to an eminent psychiatrist if we are not able to diagnose the problem. |

yes they can,most of minor psychiatric problem initialy present to mbbs doctor in bd.if they able to understand problem,they can provide.clear knowledge of psychiatric drug is mandatory for his prescription. |

And I would add that if we agree with the majority of the criticisms to DMSV, it is better to the patient having a psychiatric drug prescribed by a non-psychiatric physician. All Best, Joey

Absolutely YES The non-psychiatrits have the right to prescribe these drugs ( ratioanally) . Anti Depressants and other psychiatric medicines have a definite role in the management of chronic pains of various etiologies like Fibromyalgias, Chronic mechanical backpains, CRPS etc Adding a mild anti depressant is very helpful in these cases and doen't need a referral to a psychiatrist. Similarly there is strong evidence that Fluoxetine ( a SSRI) in combination with Physiotherapy enhances motor recovery after stroke. Again one doesn't need a psychiatrist to prescribe this medicine to post Stroke patients.

Yes as above. As long as you are doing more good than harm and prescribing within your competence then medication for neuroses and psychoses reasonable. Whilst we need Specialists, Medicine also needs Generalists . My view is that every District General Hospital should have at least one General Medical Physician. Psychiatrist should also look for organic causes of mental illness. I had a teenage girl with a florid psychosis who was cured when her hypothyroidism was treated ! |

when I was in final year of my MBBS, Our Psychiatery Professor used to say " The best help you as a physician (non-psychiatric) can provide to patients with psychiatric disorders is to REFER them to us (Psychiatrists)". I kept this in my mind for along time, when started my own practice, but later got the realization that it may not be possible in every case. Considering the scarcity of good Psychiatrists in our community (Pakistan), as well as coming across the widespread EPIDEMIC of Depression in population! Also learning the role of Antidepressants in many non- psychiatric conditions as Described by Dr. Rathore, i started prescribing anti-depressants to my patients. Which was very helpful for the patients too. In my opinion General Practioners working in areas where Psychiatrists are hard to find should have a sound knowledge of the subject and be able to treat general disorders relating to Psychiatry.!
