Nicotine dependence and illness severity in schizophrenia

An interesting recent article in the British Journal of Psychiatry entitled 'Nicotine dependence and illness severity in schizophrenia' (Krishnadas R et al, BJP 2012, 201:306-312) showed that people who smoked with schizophrenia are more likely to also have high PANSS (positive and negative syndrome scale) scores (i.e. more severe psychotic illness), and be young, male and have higher doses of antipsychotic. Of course from this study you can't assume causality (for example we can't say smoking causes worse schizophrenia or than schizophrenia causes more smoking) but it is an interesting association. Any thoughts on why this might be? And thoughts on how we as doctors might help our patients with schizophrenia stop smoking. I always find this quite a difficult one, because much as I know smoking is bad and I would love people to quit, it often seems the least of someone's worries when they are in crisis. |
