The persistence of polio in some countries is being branded a 'global health emergency' by an Independent Monitoring Board set up to tackle the work of the Global Polio Eradication Initiative. According this BMJ article, only 1% of cases remain but it is proving difficult to achieve total eradication.

Do you still see new cases of polio?

I am right in the middle of a polio endemic zone in India as my state is one of the four states in India that still has wild type polio virus infection. There has been reports of cases though I have not seen a polio case. In 2011 till now, one case has been reported, that too from my state (See here).

I think there are a few reasons why it has proved difficult to eradicate, at least in India.
  • High population density and corresponding problems of sanitation and migration. 
  • Resurgence of Type 3 polio virus after OPV against only type 1 virus was introduced.
  • Vaccine derived polio viruses - it's a great set back to the vaccination programme once a vaccinated child develops polio. Parents become suspicious of the vaccination program.
  • Certain misperceptions regarding vaccination. I have heard parents refuse or even hide their children when door to door vaccination programs are undertaken.
  • Logistical problems.

About 1000 cases of polio recorded in 2010.  we can not eradicate  polio  from polio endemic  countries unless until we know the geno type of polio virus in the effected zones. in some countries geno type 4 cases have been recorded and  the vaccines used  are meant for geno type 3. there are major flaws in vaccination drive which are the sole reasons of its` failure so far.

I felt very ignroant reading this thread because when Sabreena asked if anyone still saw patients with polio I didn't think people would say yes. Maybe that is because I live in London and think it is a historical disease. But I do know it is a terrible disease. Terrible too that mths persist among parents about getting their children vaccinated when the risks of getting the disease are so much higher.

I agree with Tariq; there might be 1000 cases of polio in Pakistan each year. Although no health authority will admit this figure. These will admit figure of 10-12/year.
I see one or two cases each year. Even just few day ago I saw one child. She was 5 years old from remote rural area. She presented with dragging of left leg while walking. Size was still normal. There was history of fever 3 days ago of two days duration. Diarrhea was still ongoing. I rang up local health office. They came and got this child admitted in the hospital. They ordered stool testing from central laboratory.

There are still small pockets of remote areas where health facilities could not be provided. Then there is another very big hurdle. These are the war zones in North and South Waziristan of Pakistan where Taliban and American drones are fighting each other. This population is highly mobile; reaching all corners of Pakistan; This is mostly without and vaccination. Presently this population is a depot for transmission.

Another funny and sorry incident. Not much reported in medical press. American CIA arranged a fake polio vaccination campaign through an NGO in the area where osama bin Laden was residing; to get the sample of DNA of his children under the bluff of this. You know! they succeeded to get the samples. Campaign was immediately dropped.

Dr. Shakeel Afridi was conducting this fake campaign; he is in jail now. Fate of sample is not known.

Moral:   Even Osama wanted to vaccinate his children.

The fake polio campaign was in the media here. By fake does it mean there was no proper part of it at all- or just that it was used to try to get the DNA sample. Was there either no need of it or did kids actualy get vaccinations and if so did they not need them? Sorry if these are stupid questions but I did think this was interesting. On what grounds is the doctor in jail?

Fake means; no actual vaccination was used.
Secondly kids in cities are vaccinated 4 times a year by government each year. This is a vigorous campaign.
This campaign was never registered with any health authority.
Dr.Shakeel is in age for espionage; acting against your own country, in the aid of foreign country; knowingly that this campaign is intended to get DNA sample only. He is a spy and is being treated as spy.

Did the kids need vaccination? Well; WHO is running this campaign twice a year for 3 days in which all children who are under 5 years of age get the drops; irrespective of the fact whether they have completed the schedule or not. Just to include every one. No harm in it. So this is difficult to know whether these children completed their vaccination earlier or not.

Main thing no vaccination was used.
Secondly only this fake thing was running in the surrounding area.
Once the samples were collected, this was interrupted suddenly.
This Dr. Shakeel is no junior doctor; rather he is quite senior government official.

The Medecins Sans Frontieres ( doctors without borders) International President Dr Unni Karunakara  in his message of 14 July, has strongly condemned the use of medical aid for military objectives and spying, which was exercised by the America in Osama Bin Laden case.
