May I introduce this into the discussion? http://www.guardian.co.uk/society/2012/feb/08/how-doctors-choose-die?INTCMP=SRCH "How doctors choose to die" The article is written by a US GP and academic. I don't know how well it reflects what US doctors do, or if it transfers to the UK, but it claims that many doctors reject intensive anticancer treatment in favour of a shorter, better end. And raises the question of why our patients seem resistant of that as an option, clutching onto treatment that can offer a possible few weeks or months of longer life, at the cost of severe side effects and much gold.
