This patient received renal transplant 4 years ago, what is the most likely cause of his facial appearance ?

Looks like a rhinophyma, but since he had renal transplant,could it be a ??? Lymphoma???
Is the reason that he needed the renal transplant significant?

It is a Rhinophyma  due to prolonged Cyclosporin therapy given to renal transplant patients as immunosuppressant

The picture depicts what is known as 'Acne Rosacea'.
It might, in the given patient be due to the continued immunosuppressive therapy.
An established & most economic way to treat this condition is DDS (dapsone) 100mg daily or twice daily for two to three months & the lesions should start disappearing within two weeks.

Rhinophyma is a descriptive term for a large, bulbous, ruddy appearance of the nose caused by granulomatous infiltration, and hypertrophy of the sebaceous glands of the tip of the nose commonly due to untreated rosacea
Alkohol is mistakenly attributed as a cause of the disease, but heavy alkohol consumption  does aggravate the condition and consequently it is more common in alkoholic patients

The most recent treatment consists of paring down the bulk of the tissue with  carbon dioxide laser, and allowing the area to re-epithelialise, Sometimes, the tissue is completely excised and the raw area skin-grafted

Formerly Rhinophyma was used to describe a severe form of acne rosacea, causing the shape of the nose very big & ugly like the horn of a rhino. The given picture cannot be called rhinophyma! Please remember, even after CO2 cautery the lesions tend to recur which is not observed after treatment with dapsone, the most cost effective treatment.
Other treatment modalities which are not 'curative' are intense pulse light, photodynamic therapy, or isotretinoin.
You will not find reference about dapsone at wikipedia. It is the only known curative treatment mentioned in the textbooks of dermatology.
