Should 'clinical excellence' be rewarded?

There was an article in the BMJ  this week regarding financial incentives to change clinical behaviour and the consequences thereof - my take home message being that I'm not sure its that good an idea. Yesterday by chance one of my colleagues was talking about the UK clinical excellence awards scheme for academic GPs and consultants which aim to reward clinical excellence defined as being service above the level expected in a number of domains including teaching, leadership and research. I'm interested to know if similar schemes exist elsewhere in the world and whether as a general rule people think they are good ideas?

I have that in China they are rewarding each published research.

As a whole I am not a big fan of these kind of strategies.

I think going down this route may satisfy some people but for me looking into the box so to speak i think it could cause the same situation that introducing targets has caused. People I worry will start to care more about the rewards they could get instead of the actual patient care they give. Yes this would hopefully never happen but I still worry about the possibility...

We have been asking how competitive are you. This would make the rat race of medicine even worse in my opinion. Yes, the Queen's Birthday awards which have as Orders of Australia etc (we abolished knghthoods, garters and the like a while back). The doctor next to my office has an AO and one the floor below.

As for rewarding carrots I am not in favour.

I thought our rewards were our salaries and the gratitude of our patients and colleagues. If the former is adequate and the latter freely given, who needs more?

Yes. 'Clinical Excellence' must be rewarded.

While all doctors aim to do good work and many actually do good work, a number of NHS doctors work above and beyond their call of duty and work to a clearly demonstrated excellent standard.

These are not by targets, CEAs are by a kind of relative performance. Original pay structure was designed in a way that doctors' excellence can be rewarded.

Some doctors try to achieve these awards by clever 'proforma' tactics and they should be identified and stopped.

It is a shame that in some trusts the optional/discretionary points for old contract SAS doctors have been abolished and the money distributed evenly to everyone - promoting the least common denominator attitude. The new SAS contract has no discretionary points and pay progression has been handed over to managerial discretion - a bigger crying shame.

For poor performance stuff like suspension, retraining, referal to NCAS/GMC/performance-review etc are options; for regular good performance normal pay (and additional pay for extra lists, priv practice) are options; for excellent performance CEA is the option.

I could go on and on.
