Hey. Help me please. 

Man, 57 years, smoker with dyslipidemia, hypertension, dilated cardiomyopathy, paroxysmal atrial fibrillation, valvular disease and alcoholic liver cirrhosis, abstinent for 3 months and underwent repeat paracentesis fortnightly.
Refers to the urgency on 26 October 2011 with the following table:
Since a week ago with diffuse abdominal pain, periumbilical, moderate, with no aggravating factors and soothing, accompanied by a progressive increase in abdominal girth associated with disorientation and malaise, decreased urine output. He denies fever, nausea, vomiting and blood loss.
At the hospital he had hypotension refractory to fluids and anuria requiring dialysis support.

Heart Rate: 150 bpm
Blood pressure: hypotensive (90/60mmHg)
Respiratory rate: 44 cpm
Temperature: afebrile

The possible diagnoses are:
Spontaneous bacterial peritonitis
Portal vein thrombosis
Hepatocellular carcinoma C
Cholecystitis

Medication:
Furosemide 40mg (2 + 1 snack fasting)
Spironolactone 100 mg (½ comp. Am)
Nitroglycerin 5mg TD from 8 to 20h
Propranolol 10mg (3x day)
20 mg pravastatin (a comp. At night)
Pantoprazole 40mg (a fasting J + 1)
Iron 256mg (2x day)
Ramipril 1.25 (1 + J PA)
Lactulose (2 tsp 2x day)
ASA 100 mg (1)
