You might like to read this blog about how 'bad' we consultant's are about clinic letters and discharge summaries. I agree completely with this - bad communication = bad care but from my perspective on the other side it is system's of work and not individual clinicians eg you can do whatever you like with the letters but if you can't email / fax /post it and get it read what does it matter? My rather tongue in cheek response might be

Dear primary care colleague
please kindly send me an automated summary of every drug the patient has ever had and every condition ever entertained
please bury your clinical concern somewhere in the middle of these 7 pages
encourage the patient to exercise choice so that all relevant tests are conducted at different hospitals
very kindly do all the blood tests but not on a system that I can access
automate the practice's telephone system so that I can try to work it out?

anyone add any more

Nowadays, especially when there has been a remarkably high incidence of medicolegal claims against medical practitioners, a properly maintained clinical record forms the only weapon / tool to defend ourselves strongly!
The notes should be written clearly, avoiding abbreviations, should contain a details regarding our positive or negative findings alongwith a detailed history including previous illnesses , habits , & a good family history & important relevant investigation done, & of course every details about our treatment advice.
