Clarithromycin verses Azithromycin

Off at a tangent, a bit, but we recently ahd to decide which to recommend as our long term macrolide for anti-inflammatory therapy and exacerbation reduction in non-CF bronchiectasis. Clearly it's an evidence vacuum, so we had to make a pragmatic choice. We went for Clari as it's much cheaper. I've only prescribed azithromycin to patients with CF in the past 5 years.

1-Azithrocin is cheaper than clarithromycin in Pakistan. 2-It is once a day 3-It covers all micro-organisms, covered by clarithromycin, and others including salmonella typhi and parathypi. ( this is the point I request elaboration). 4-In respiratory tract infections, 3 dose in 3 days covers for next 7-10 days. 5- Still why clarithromycin? |

if the practitioners themselves do not try to evaluate / judge the efficacy of a drug they prescribed on daily basis then otherwise face the interpretations of a drug the business enterprises put before you. |

In cases of LRTI, where the macrolide is not the drug of choice, Clarithromycin offered better symptomatic relief than Azithromycin. Proprietary brands of Azithromycin, of course, happen to be cheaper than that of Clarithromycin. |

We don't use Macrolides as first, or second line treatment for LRTI, so the question is mute for us. We *do* give macrolides for their anti-inflammatory activity, so in this evidence free zone we go for the cheaper option. The spectrum of coverage, length of treatment etc are all irrelevant. |
