The Department of Health wants to hear what you think (honestly) about how to improve the lives of people with long term conditions. If you know of any good practice or have any ideas on what could be made better for the people who work with those who have long term conditions, the department wants to hear from you. The department is developing a cross government long term conditions strategy and your comments will be fed into the process. So here's the latest question:

What can be done differently to help patients with long term conditions stay as well and as independent as possible?

There are many ways we can help the patients with long term conditions or chronic illnesses. We can help them live in better way by,
1) Providing them the much needed financial help for a longer duration, as long as they may live.
2) Provide them a good psychosocial support, organized by the local healthcare personnel.
3) Provide them a good & healthy living environment & a shelter whenever feasible.
4) Provide them routine healthcare visits, which could be implemented via local expert clinicians.

neurological disorder mostly chronic condition.without pharmacotherapy,they needs sympathyand psycological support and treatment ,with rehabitalation.planned physiotherapy, cognitive behavioral therapy improve thier outcome. economical support and good caregiver is very essential for thier improvement.

A good start would be recognizing that the medical model we currently employ that focuses on acute care was developed when injuries and infectious diseases were what filled to consultation room. Now it is filled with people with chronic diseases, mostly diabetes and cardiovascular diseases. There is no shortage of information as to what works in the care of such persons, only a shortage in the medical communities' adopting the necessary strategies. Ten minute visits four, five, six or more times a year will not work. First and foremost these people need the skills and education necessary for self-care and monitoring their diseases. That requires a team of people with the necessary knowledge and pedantic skills to transfer that knowledge and those skills to patients and to provide the support needed for them to incorporate these into their daily lives. We physicians need to understand that whereas we treat people with acute illnesses, we only can consult with people with chronic illnesses. They are doing most of the treatment; we only intervene when something goes wrong. The goal of self- monitoring be it in diabetes, congestive heart failure or hypertension is to prevent or minimize these occasions. That is our goal- to keep them healthy as possible and out of our consultorium and hospital.

Chronically ill patients require special medical attention on a regular basis.
This affects the patients ability to remain independant as carers are usually needed to help facilitate the regular visits to hospital.
Recent studies conducted by UK  Dept of Health suggest that Telehealth is a new technology that can assist in better practice for chronically ill patients.Telehealth provided in a patients home enables them to take readings of their vital signs,blood pressure,blood glucose,oxygen levels etc on a daily basis without assistance.

This new found independance has several flow on effects,firstly reduced mortality rates(results from one of the most complex and comprehensive studies ever undertaken by the UK Dept of Health ) show a reduction in mortality and a reduction in hospital admissions.Having constant monitoring available at home also provides the patient peace of mind and reassurance.

The technology is available now and can extend to live video connections with the consulting physician and the patient at home.I am a true believer in the positive effects technology has to offer in the advancement of traditional medical practices.

I think that streamlining health services such as clinics, imaging and other diagnostics around the patient (especially those with chronic disease) is vital. This has been practiced with good results at the Virginia Mason Hospital and Medical Center in Seattle. Although such a shift would require significant physical as well as system restructuring of the health care environment, it would be well worth it in the long run.

A litany of chronic and idiopathic conditions can be eliminated by changing diets. First it needs to be recognized that the intestinal bacteria needs to be in good condition. If the lactobacilli are not in good order, candida takes over and makes the gut porous with the result that undesired intestinal contents enter the system. The effects on the immune system by this material are uncountable and result in a lot of business for the medical profession.
The gluten in wheat, rye and barley has increased over the years because of the selection of varieties of these grains to make better bread. The increased gluten has a damaging effect on intestinal villi resulting in reduced adsorption of nutrients which in turn causes a litany of idiopathic conditions. A similar situation exists in relation to A1 Beta Casein contained in milk from Holstein, Freisen and Aryshire cows. It is related to type 1 diabetes if given to babies under four months of age. Guernsey and Jersey cow milk contains A2 Beta Casein which doesn't seem to cause problems.
Lactose is another potential problem causer that is not adequately considered as is the fact that milk is a cause of osteoporosis.

More psychologists would be a great starting point.  

The psycholgical burden of chronic disease is often underestimated.  My most difficult patients to manage all have psychological issues, and I have access to 0.5 PA per week of a psychologist, and she's meant to see all the CF patients, not the other chronic diseases I see.

The waiting list to see the clinical psychologists at the moment in my region is 18 months.  

Clinical psychologists must go through a primary degree, a higher degree, postgraduate training and clinical training.  They are very good at what they do.  Why aren't there more of them available to help our patients come to terms with the psychological burden of their chronic disease?

I think a 'medical model' of chronic disease management will never work in the long run because it will be too expensive. We need to spend more money on prevention and lifestyle modification and then for those who still get the disease they need to be supported to manage their own condition - telehealth has a role but it should be used for those who are the most complex / difficult to manage. The weightwatchers model has a lot going for it as far as I can see -peer support and advice and a fee if you turn up!
