What are antibitoics any use for?

A study in JAMA says amoxycillin doesn't improve symptoms in people with uncomplicated sinusitis. Even a purulent nasal discharge and pain in your sinuses is not relived any faster with amoxycillin, Since I recently had sinusitis and took amoycillin as though it was going to save my life (and could have sworn it did) it made me wonder what antibitocs are any good for in terms of common, self limiting conditions.

The study included 160 patients with clinically confirmed disease (purulent nasal discharge and maxillary pain or facial tenderness) who were randomized to receive either 1500 mg/day of amoxicillin or placebo for 10 days. In addition, all received a supply of symptomatic treatments (e.g., acetaminophen) for use as needed.

Patients' assessment of improvement in 16 sinus-related symptoms did not differ between groups at day 3 or at day 10. (While symptom scores did favor antibiotics at day 7, the authors judge the difference to be "too small to represent any clinically important change.")

So is there a place for antibiotics for anything common and self limiting?

Strep sore throats are self limiting - but they can go on to cause Rheumatic fever if you get the righth bug in the right person - should we leave those untreated?

This Thread is Rhetorical in one aspect,  It all depends on the patient.  Left untreated Sinusitis can cause Severe problems with Teeth, and Eye problems,  I suffered with severe sinuses for years, still do from exposure to toxins in workplace.  Around 4 years ago I had to have a Dacryocystorhinostomy due to closing of tear duct from repeated sinus infections.  The procedure worked 100%, and I now a regimen for sinuses using a Neti Pot,  and saline irrigation as well as the product Alkalol for irrigation.  I also incorporate Herbs- Echinacea and vitamins into my regimen.   As a side note Ceftin is a better antibiotic for sinus infections, better tissue penetration, less side effects,  Clindamycin works well too.  So some people do benefit from antibiotics,  and We must always remember if eyes become involved from sinus infections, the brain is a hop and skip away, and severe infection \\ death not far behind,  also compromise of the airway in extremely severe cases, sepicemia, etc.   Bacteria are becoming resistant so sometimes multiple courses are required to cure a sinus infection.      DuaneF

Personally, my experience suggests that in patients for whom the use of tetracyclines is not contra-indicated, doxycycline is far and away the most effective antibiotic for treating acute sinusitis.  Amoxycillin is worse than useless for that, and for strep tonsillitis, as well as being one of the worst offenders for causing C DIff.

I understand that the so-called evidence base for the use of steam inhalations to relieve symptoms of sinusitis suggests that they are not beneficial.  I am old enough to think that my 30-odd years of experience in General Practice and the anecdotal evidence I have noted is much more convincing!

Doxycycline seems to be the business when I get sinusitis, I agree.  In fact, I get very rapid response to Doxy.  Makes me wonder if a large single dose would be as effective as 5 days....  I sniff a study....

Dundee Chest,   as a side note, my reasearch says that Bacteriostatic antibiotics like Doxy work best,  also there are some novel uses of Saline rinses with Clindamycin in the rinse for nasal irrigation that seem to work somewhat.   Can be somewhat irritationg to nasal membranes, must be buffered with sodium Bicarb,  DuaneF

Last year,  I tried a new treatment,   Saline  irrigation,   4 times a day,  but Am irrigation I use 4 drops of Grapefruit seed Extract,  very Viricidal, and antibacterial.   Seems to work well, has side effect of shrinking swollen sinuses, and breathing becomes better from the extract.   Yes I try alot of stuff on myself,   DuaneF
