Yes, Dr. Sadian, it is a large ostium secondum atrial septal defect causing right atrial enlargement as is evident from the echo pictures especially the transesophageal, which is showing left to right shunt or pressure gradient.
In view of her management with surgical closure of the defect we need to know the quantitative analysis of pressure gradient. Her chronic CHF is secondary to pulmonary hypertension.

This lady had her ASD closed percutaneously about 3 months ago with an unmbrella type device. I saw her on Thursday and she is very well. She no longer has severeTR (her neck doesn't pulsate!) and whe no longer has periprheral oedema . On echo her right heart size is almost normal.

I am satisfied with the management of the patient & happy to know that she has improved symptomatically & her CHF is under good control.
