I noticed a study presented at the European Association of Diabetes meeting suggests that glyburide use is associated with more ACS presentations than gliclazide. http://www.theheart.org/article/14544

I've got no experience with glyburide - does anyone prescribe this and is this study result explained by confounding?

Glyburide in India is available as Daonil.

This is also called as glibenclamide. Glibenclamide is the most potent oral hypoglycemic agent with longest half life & cumulative toxicity & is known to be associated with increased episodes of ACS. Hence should not be prescribed in elderly considering the fact that most of the sudden deaths in diabetic patients are due to hypoglycemia.

apart from CV risk profile, they also differ in tendency to cause hypoglycemia, maximum with Glyburide ( glibenclamide) and least eith Glimepiride.

i feel the reason behind could be effect glucagon suppression, least with glimipride and max with glibenclamide.

this could also be the reason behind why glimepiride fails to acheive glycemic control in T2DM patients > 10 yrs old on therapy, in whom glibenclamide score better.

I find Gliclazide the most safe and user friendly sulfonylurea.

I has a short half-life, but can be given once-daily with its extended dose release tablets, and have a wide dose range. And it is not expensive

There is also good and long-term clinical trial data supporting its use (the ADVANCE Study).

So, it is my number one sulfonylurea.

All Best,

Joey
