ivc filters for thromboembolic disease

my colleague and i spent about 30mins debating this case yesterday

70 year old man
previously fit and healthy
dvt and pe post flight to us last may
6 months warfarin very breathless - echo mild rv dilatation, right heart cath 3 months in normal pa pressures
warfarin stopped at 6 months and then further dvt and pe 8 days later - now on lifelong warfarin
had episode about 6 weeks ago with further leg swelling and chest pain for 1 day, didn't seek medical advice and no inr recorded
would you recommend an ivc filter in his case? We both had different ideas so I'm keen to hear what others think?

The above mentioned patient can be considered for IVC filter due to failure of anticoagulant therapy, and risk of recurrent PE.

IVC filters can be used to try and prevent pulmonary emboli. Inferior Vena Cava (IVC) filters can be used in cases of documented pulmonary embolism (PE) where anticoagulation is contraindicated or has failed. The efficacy and safety of vena caval filters in the prevention of pulmonary embolism in patients with proximal deep-vein thrombosis are still a matter of debate. IVC filters had no detectable impact on mortality and are associated with an increased risk of recurrent deep venous thrombosis. They can be used in patients who remain “very high-risk” despite preventive or curative anticoagulant treatment. Indications for filter placement are restricted to contraindications to or failure of anticoagulant therapy.                                                                                                                                                                        Low molecular-weight heparin is as effective and safe as unfractionated heparin for the prevention of pulmonary embolism.
