I'm in the throes of preparing a talk on palpitations for a group of primary care physicians and I'm doing my ectopy slides because I reckon these (and sinus tachycardia) are the commonest dysrhythmia

I'm in the throes of preparing a talk on palpitations for a group of primary care physicians and I'm doing my ectopy slides because I reckon these (and sinus tachycardia) are the commonest dysrhythmias seen in primary care. What features prompt you to investigate further?

I did not understand exactly your question Sadian. You mean to examine farther the ectopic beats or the palpitations in general ?

Symptoms.
So many are symptom less, or merely noticed, causing worry and needing reassurance.

Hello. I did some reading on this recently as I wanted to know the answer too.

 Basically ectopics are atrial or ventricular. 

Ventricular (PVCS) do not pose a health risk unless there is a history of heart disease  in which case you should refer for investigation. if no heart disease then prognosis is good although if occur during exercise then should investigate too. 

Atrial ectopics are v common and may occur with increased pressure on the atria such as cardiac failure or mitral valve disease, although can be caused or exacerbated by caffeine or alcohol.


any ectopics which cause chest pain , Shortness of breaeth or sycope (esp. exertional syncope) need urgent invx. 

frequent ectopics - invx with holter monitor
infrequent - need event monitor
bloods etc.

low risk patients with no worrying features can be reassured!

I agree with the above but I wonder what you feel about exercise / recovery induced ectopics? Do you think these indicate a worse prognosis and should always be investigated?

I think yes. Say the truth i feel that something is going wrong or will be in the near  future. i always investigate them at least with an interviu, clinical examination and assesment of risk factors.
 After you finish your presentation Sadian poste the slides and heare on doc2doc. I believe many of as would like to learne something more or take a look again on these interesting subject.
