Twisting Tournequets, Richard! Why not try simple airway control techniques first? Yes, a full airway control kit would include a Minitrach or similar, but please: explain to us now the acute situiation in which that would be needed? And, have you ever inserted one? This will be useful when you have to explain it to the coroner. As it happens, I do have one in my car kit , and No! I haven't put one in either, and I'm b*gg*r*d if I'm going to reach for that unless I have tried first every single skill I have, in anaesthesia and pre-hospital trauma care, with my hands, with supra-laryngeal airways and endotracheal tubes. I only have one because I know how to use it, and have practiced on animal models. If you haven't, then prepare your statement to the coroner now about how you stabbed the casualty in the neck. Practice weeping in the witness box and saying through broken sobs how you were trying to do your best, even though you had no idea what you were doing and had never used the technique before. KISS! Keep It Simple Stupid! On tournequets. The best treatment for acute bleeding is: 1/venous bleeding (EG damaged varicose vein) Elevate the limb. 2/ Aterial bleeding (IE it spurts) Firm pressure over the wound. Major bleeding may need a tournequet, and for the squaddie in the desert, whose comrades may be pinned down by enemy fire, a combat tournequet that may be applied with one hand may be, and has been, life saving. But it is rare that we are pinned down by enemy fire in tjhe UK. In Afghanistan, the use of chitosan bandages, to arrest catastrophic bleeding has been successful, and we had some for the Olympics. $40 for a 3"x6' bandage,3 year shelf life. But how often wil you have such an injury? John |

As others have said you should not carry any equipment you have not been formally trained to use and you are not using on a regular basis and I am sorry but an ALS course you did in 1999 or a BASICS course from 2002 doesn't count - skills atrophy within 6 months if not used regularly and are lost with a year or so in some cases (and I am speaking as someone who used to be a BASICS instructor and a qualified ALS instructor). If you want to carry something carry an automated defibrillator, otherwise, as JohnD has said, stick to the basic ABCs and let the pros (i.e paramedics) do their job. |

SP services are selling Olympic ready fully kitted ALS/RSI bags for a discount (though as I haven't checked the itemised cost of each item, I can't personally vouch for the price). The list of contents would be useful for anyone updating their personal emergency medical bag though, and is available on the SP website. |

Indeed. £30 for a First Responder bum-bag and £500 for BLS and ALS back packs. See my post on previous page. But please also see that post, on competence and the actual need for kit while waiting for the ambulance. John |
