This week I've found myself programming an excel spreadsheet to aid my consultant colleagues in establishing a database of patients recently diagnosed by autism to allow them to calculate staff costs and time delays involved in the diagnosis pathway.

I was interested to read in BMJ Quality and Safety that "Doctors in training have a desire and perceived ability to contribute to improvement in the NHS but do not perceive their working environment as receptive to their skills" .

Based on their questionnaire of junior doctors only 3.4% said they had never acted in a leadership role. Incredibly "91.2% of respondents have had ideas for improvement in their workplace; however, only 10.7% have had their ideas for change implemented".

  • Are junior doctors an untapped resource?
  • Should the NHS do more to involve the future consultants in leadership at an early stage?

Hugely, yes. Contrasting the approach in Europe, where nurses are trained to & expected to take bloods & cannulate patients, myself & my colleagues find ourselves doing just that for around 50% of our working lives. The other times are filled with notekeeping, updating the list & chasing results that, more likely than not, will get there when they get there.

Suggestions are rarely investigated & implemented, in my experience. It becomes something to moan about rather than something where action can be taken.

Junior doctors don't think they will be listeened to and often with good reason as medicine is still a hierachy and you don't want to annoy your seniors with great ideas for quality improvement. This might imply you think that quality is not as good as it should be and your consultanrt may not find this endearing.ALso junior doctors are busy and ofetn worried about the job they are doing, which makes them less able to feel coinfident about the ideas they do have. And have ideas they certainly do have- often very good oones that can improve patient care. They should be enoucouraged to share these ideas - it is harder to find a forum for them to do so with the shifts they work and the loss of the firm structure. So maybe Trusts need to hhkn how to formally harness the talent of juniors outisde of everyday clinical duties.

Yes, as long as you substitute "untapped " by "unwrapped " please.
