Hookah, also known as shishawaterpipe, or narghile is a multi-stemmed instrument for smoking flavoured tobacco in which smoke is passed through a water basin before inhalation to purify the smoke and minimise the effect of nicotine in the lungs and circulatory system

This social habit is becoming widespread now specially in London, among both men and women in most coffee -shops where people socialize and spend time with friends and relatives

Despite the fact that passing smoke through water could reduce its direct effect on the bronchial tree - as some people claimed that respiratory tract infection, and COPD is much more common upon cigarette smokers rather than Shisha users, but it is also possible that microorganisms and viruses , such as  Epstein–Barr virus and herpes simplex, can be transmitted directly from one person to another, as all companions use one pipe at a time.
I think it is important to draw the attention of Shisha users to the possible effect of direct person to person infection as a result of this common social habit

My underline.

alaminium,
Is that really the purpose, because a low nicotine smoke must give the smoker less 'satisfaction'.  And purify?   The Britsh Heart Foundation says that shisha smoking can be more harmful than cigarettes! http://www.bhf.org.uk/heart-health/prevention/smoking/shisha.aspx
They base this on the duration of a shisha session, warning that an hour on one might be the equivalent of a hundred cigarettes!  My favourite research resource, PubMed, has only 23 papere on "shisha risk" and most ae on people's lack of risk perception!
I'm sure you are not advocating this as a better alternative to cigarettes, just warning of yet another risk.

It depends on the technique John, some Shisha users inhale the smoke deeply in their lungs, and this group are the most affected and might be more harmful than cigarettes, while others keep the smoke only in their mouth and nose, but they could be affected directly by transmitted bacteria and viruses from other users

I am not defending the Shisha group, but drawing the attention of the possible risk and consequences of this commonly spreading smoking habit

During the first Gulf War I was in Southern Saudi Arabia. A group of us travelled further south to the Yemen and in the middle of nowhere (and I mean that virtually literally) we came across a meeting house where elderly men were imbibing on such pipes.  One or two people tried them but I did not. A Doctor travelling with us was concerned about TB and other infections being passed by their use. I am glad I did not use one. They were just passed around amongst whoever wanted to use them and I really did not feel encouraged at all to imbibe.  Of course there were no illicit substances in them - being in Saudi the penalty for posession of drugs is death as it warns one in big red letters on the landing card.

That is right, I have one of my patients, who is a regular Shisha smoker badly affected by mouth ulcers due to herpes simplex virsus infection.

Many microorganisms are transmitted through the saliva between the group users, most common Epstein–Barr virus, which can cause infectious mononucleosis, Hodgkin's lymphoma, Burkitt's lymphoma, and nasopharyngeal carcinoma. There is also some evidence that it is associated with high risk of certain autoimmune diseases, particularly dermatomyositis, SLE,
rheumatoid arthritis, Sjögren's syndrome, and multiple sclerosis

Increasing awareness of dangers of Shisha smoking is important. Shisha houses should have prominent notices that say :-  “Smoking shisha can kill” - just like that on a cigarette packet.’

For the people of hookah smokers, you have all good reasons to opt it over the cigarettes. When you compare the shisha with a cigarette, you are bound there are less health hazards as compared to the latter option. With cigarettes, you are bound to damage your respiratory tract infection and COPD, which is not the case with the Shisha smokers.
