Plumbism? He works in a lead smelter and has lead nephropathy causing hypertension. No he works in a battery factory or is it a copper lead and zinc mine at Mt Isa Mines, Queensland and he has a saturnine personality.

The clue to the answer is the fact that the pateint is hypertensive, the gingival hyperplasia which he has is due to the antihypertensive Nifedipine
