The NHS is full of hard decisions and this is one of them, but compare with the climber with a broken leg, who has brought his injury upon himself.  Will you insist he 'goes private' even if he developes compartment syndrome and fat embolus?  Will you deny your patient obstetric care as an NHS patient?   Even harder, for them, the women with faulty breast implants who now want them removed, and no dout replaced with better ones.
