The day before I was due to go away , water came pouring through the ceiling from a leak in our attic roof. With a variety of "tools" and a ladder (about 30 feet) I managed to clear a blocked gully on the roof. If I had damaged self or house that would have been my fault and I would have had to seek outside help. Yet if I had done something well within my competence and training , such as prescribing a course of metronidazole for myself for a dental abscess I would be severely criticised. Again , if the treatment did not work I would need to seek  professional help.and again the responsibility for problems would be mine. I think this issue does need repeated airing. The only rapid response with the BMJ that I have ever had rejected was one suggesting that there are sometimes advantages in treating self and influencing the care of one's family. It seems the one big area where there is an obsession with a one-sided argument with no attempt to look for the balance.of pros and cons and to have an evidence -based approach. End of moan.
