Right heart strain, with the classical S1Q3T3 pattern.  Although often associated with PE, it can be caused by any form of acute right heart strain.  Not quite a tachycardia, I note.

The ECG shows First Degree Heart Block + Left Anterior Hemiblock

In fact the space was too small in the screen to show the chest leads, where you could have seen the RBBB. And if you add the 3 lesions together ( First degree heart block + Left Anterior Hemiblock + RBBB ) you get a Trifascicular block, which is the diagnosis
