After Action Review (AAR) is a technique created by the US military to debrief its personnel after incidents. AAR is being rolled out across University College London Hospitals, as a structure for frontline quality improvement. It is a no-blame process for analysing incidents and near-misses. It focuses on establishing what was expected, what actually happened, why there was a difference between these events and what lessons can be learnt. It is conducted by a trained facilitator and involves the staff directly involved in the event. It should take place as soon after the event as possible as that is the point of maximal learning. Critically the process allows staff to take ownership of lessons learnt and implementing change.
