1. The Lancet have published two papers stating that the NHS is not adequately equipped to treat patients with mutiple health problems. A problem being that most healthcare research focuses on single diseases, when patients suffer from multiple diseases. How should services be designed to accomodate this?

2. Criticisms aimed at authors of the APA's DSM5, which is due to be published in May 2013, has led authors to add a footnote “to clarify the difference between normal bereavement associated with a significant loss and a diagnosis of a mental disorder.”

 

3. Experts from the Society for Radiological Protection have called for cosmetic laser treatment to be regulated, warning that its misuse can result in skin burns, blistering, scarring, and permanent blindness. Cosmetic laser treatments are being used to remove hair, tattoos, birthmarks etc.

I still do a lot of general medicine.  My take last night varied between mechanical falls, sepsis, asthma, cardiac chest pain, PUO, cellulitis, AKI, confusion, and a gaggle of upper GI bleeds.  Most of the patients had multiple problems, co-morbidities, and complex PMH - this is the nature of our general take.  

Laser beams?  Sharks?  With Laser beams?

Many thanks Dr. Matthews, for providing an excellent thread for discussion, which, in fact, I was planning to in my next postings of the  BMJ doc2doc forums, which I think is the most appropriate platform to express & share our thoughts.
Our problem is that we always forget our 'Mother' I mean the basic branch of all the medical sciences is General Medicine.
But when we review today's scenario in medical education , it would be apparent that more & more focus is being on the speciality, superspeciality & what's more, ssspeciality?
Here is the basic thing where, as I think we are going wrong & misleading not only the community but misleading ourselves! This trend must be stopped at, by higher & competent levels, & must appreciate the Lancet for taking the initiative in this most important healthcare management problem.
The authors challenge the single-disease framework by which most health care, medical research, and medical education is configured. They concluded that a complementary strategy is needed, supporting generalist clinicians to provide personalised, comprehensive continuity of care, especially in socioeconomically deprived areas.
So do I think that we are in need of general practitioners with multifaceted capabilities & versatility.

I gave up reading The Lancet after the Wakefiled disaster and an editorial that accused GPs of being responsible for the deaths of patients. I will continue to boycott it whilst the current editor remains in post.
