Its all in the history!
I am not a whiz at history taking. However, I always look forward to taking a social history. It Here are certain scenarios that tend to occur now and again

Me: Do you smoke?
Patient: *vigorous head shaking* "NO, i don't do anything like that".
Me: Have you ever smoked?
Patient: Yes
Me: For how long did you smoke and how many would you smoke?
Patient: Well...er, i quit about a week ago, and I would normally smoke around 30 a day.

Scenario 2:
Me: Do you drink alcohol
Patient: Occasionally,
Me: how much do you drink?
Patient: well, i don't drink a lot you know. I'm more of a social drinker. Maybe a couple of pints when I am out with friends, but nothing heavy.
Me: Oh okay, so how often would you go out with friends?
Patient: almost Every night <nervous laugh>, you see I live above a pub...
Me: right. How many units would you drink every night?
Patient: I dont really know how to convert it into units. perharps 4-5 pints, and maybe the occasional shot of whiskey...
 
Now i'm  not good at converting alcohol consumption into units but i'm pretty sure 28-35 pints a week is excessive.
